Annual Health and Medical Form (replaces
Class 1, 2, and 3 health forms). This form meets
Massachusetts guidelines and is to be used by all summer
camp participants. Resident camp participants
(youth and adults) need all three parts (A, B, and C).
Day Camp participants (Cubs and Kid Town) need parts A and
C. Staff members need all three parts (A, B, and C).
Volunteers (those that are helping only a day or two) need
parts A and C.
